What This Test Measures

Gold-standard mass spectrometry method for total testosterone; more accurate than immunoassay, especially at low levels.

Preparation

Fasting required (8-12 hours)
Draw in the early morning (7–10 AM) when testosterone is at its daily peak. If on testosterone BHRT injections, draw at trough — just before your next scheduled injection — for the most clinically meaningful result. Quest may report inflated values for women on BHRT; CPL or LabCorp recommended for trending.

LC/MS/MS is the gold standard methodology, more accurate than immunoassay especially for women and low-range values. For women on BHRT, this method avoids the false elevations seen with Quest's immunoassay. The TRT community tracks trough levels to ensure adequate coverage between injections.
