What This Test Measures

Elevated in congenital adrenal hyperplasia (CAH); also measured in adrenal function assessment.

Preparation

Fasting required (8-12 hours)
For cycling women, draw on Day 21 (7 days after presumed ovulation) for luteal phase assessment. If on BHRT, draw at trough. Fasting not required.

NOTE: 17-OH Progesterone is NOT the same as progesterone — it's an adrenal/ovarian precursor used primarily to screen for congenital adrenal hyperplasia (CAH). For cycling women, draw in the follicular phase (day 3) for CAH screening. Levels vary by cycle phase, time of day (highest AM), and stress. The BHRT community notes this test should not be confused with standard progesterone monitoring. These notes reflect common practices in endocrine patient communities.
