Prolactin

CPT: 84146
Hormones

What This Test Measures

Hormone produced by pituitary; elevated in pituitary adenomas, hypothyroidism, and certain medications.

Preparation

No fasting required
Draw in the morning after at least 20 minutes of rest. Avoid breast stimulation, sexual activity, exercise, and stress for several hours before testing — all can temporarily elevate prolactin.
💬 Patient Community Notes

TIMING: Draw fasting, first thing in the morning, in a calm state. Prolactin is extremely stress-sensitive — even the anxiety of a needle stick can elevate results. The community recommends: arrive early, sit quietly for 15–20 minutes before the draw. Avoid vigorous exercise, nipple stimulation, and sexual activity for 24–48 hours before testing. Do not drink caffeine before the draw. Eating can also elevate prolactin. A mildly elevated result (especially under 50 ng/mL) should be repeated under ideal conditions before pursuing workup. Many mildly elevated prolactin readings are stress artifacts, not true hyperprolactinemia. These notes reflect common practices in patient communities and may differ from standard lab guidelines.

Reflects common practices in thyroid, BHRT, and functional medicine communities. This may differ from standard clinical guidelines. Not medical advice — always discuss with your provider.

General guidelines only. Requirements vary by lab and individual. Confirm with your provider before adjusting any medications.
