What This Test Measures
Morning cortisol drawn 8-9 AM; screens for adrenal insufficiency (low) or Cushing's (high).
Preparation
TIMING: Draw between 7–9 AM, ideally within 1 hour of waking. Cortisol peaks 30–45 minutes after waking (the cortisol awakening response). Avoid vigorous exercise, caffeine, and high stress before the draw — all can spike cortisol. The functional medicine community notes that a single AM cortisol can look 'normal' while the daily rhythm is dysregulated. They strongly recommend 4-point salivary cortisol to assess the full diurnal curve. For salivary testing, STTM says stop cortisol-affecting supplements for at least one week before testing.
