What This Test Measures
Salivary cortisol measured at 4 time points throughout day; assesses diurnal cortisol rhythm.
Preparation
PREP: Stop all cortisol-affecting supplements for at least 1 week before testing (hydrocortisone, adrenal adaptogens, licorice root, pregnenolone, etc.). See STTM's list of cortisol-affecting substances. Collect samples at the specified times (waking, midday, afternoon, bedtime). Avoid eating, drinking, or brushing teeth 30–60 minutes before each sample. STTM's optimal pattern: morning at the very top of range, midday in upper quarter, afternoon around mid-range, bedtime at the very bottom. This 4-point test reveals the full daily rhythm that a single AM serum cortisol cannot. These notes reflect common practices in adrenal/functional medicine communities.
